Refer to the Settlement Agreement for complete terms and conditions of the settlement.

The settlement became effective on April 26, 2012. On or before May 16, 2012, AHS sent a postcard notification to Class Members advising them of the opening of the Review Desk. The Review Desk opened on May 26, 2012, and accepted Submittal Forms until September 22, 2012. The Review Desk is now closed. AHS is currently processing Submittal Forms received during the Submission Period and will make a determination as to each timely Submission in accordance with the terms and conditions of the Settlement Agreement.
