The settlement became effective on April 26, 2012. Within twenty days of the Effective Date, AHS will be sending postcards to Class Members advising them of the opening of the Review Desk. The Review Desk will open on May 26, 2012, and will not be accepting Submittal Forms until that date. Review Desk Submittal Forms are available by clicking here.

Claims review process

The Review Desk will open May 26, 2012, and will not be accepting Submittal Forms until that date. Once the Review Desk opens, if you made a claim between June 24, 2001 and October 20, 2009 under your American Home Shield Home Warranty that was denied in whole or in part, or if you were not satisfied with the payment you received from AHS instead of repair or replacement services, you may re-submit your claim to the Review Desk. To re-submit your claim, you must fill out a Submittal Form and it must be postmarked or emailed on or before September 22, 2012. You may obtain a Submittal Form by clicking here, by calling 1-877-433-3843, or by sending a written request to the following address: Class Action Claims Review Desk, P.O. Box 727, Carroll, IA 51401.
Be sure to completely and accurately fill out your Submittal Form. You may also submit any additional documentation with your Submittal Form that you feel will assist the Review Desk in evaluating your claim. Examples of such information include, but are not limited to, photographs, receipts, cancelled checks, correspondence with AHS, correspondence with contractors (whether or not chosen by AHS) that diagnosed or attempted to correct the relevant service issue, and a statement providing relevant history or an overview of events.
